Technically you would have to be 18 in the US to work with biologicals and to dispense medications which require a DEA number. However the vast majority of doctors will be older than that by the time they complete their eduction. 4 years of undergrad + 4 years of medical school + 1 year internship + 1-7 years residency (dependent on the specialty you choose).
As far as Scrubs is concerned, Dr. Cox is an attending physician and at the beginning of the series JD was an intern, which meant that he had just finished medical school and was doing his 1 year internship in internal medicine. The second season he was a resident and after that he became an attending like Dr. Cox. Once you are an attending you are a full fledged doctor. However residents and interns are still MDs and can still preform medical procedures and so forth, they just have not fully completed all of their post medical school training.

You have to have a Doctorate at a medical school. This takes about 12 years if you know what you’re doing.
If you start at 18, you’ll be done with schooling by 30.
