Having doctors offer affordable rates for minor jobs. Like if you are hurt, you see a doctor and pay $25 for the visit. You pay out of pocket what you would pay as a co pay. The insurance company isn't involved and you'll use your insurance less. There for money will be saved and hopefully you'll have less people being denied because you'll be using it less.

I am an Indonesian-born Indian, working as a medical resident in Australia. I was trained here in Aussie, and am now wanting to move to the USA to be with my partner. What steps will I need to take in order to be able to work as a doctor in the US?

At what point were people dissatisfied with their health care? Isn't it the expense that people were worried about? Why are we changing the whole system when opening up competition to drive prices down would allow more people to buy the health care that is needed?
If you reference the report that has us 37th on the health care list I dismiss this "report" on the basis of bias, we have the only health care industry in the world that is "free market" (for lack of a better word).
Also the profits that the health care industry makes goes towards furthering treatments and pharmaceuticals.

Would they specialise in Plastic Surgery, OBGYN, Urology or what???

I have worked in the financial services industry for 20 years. I feel nursing is my calling. I have a 28 , a 23 , a 9 and a 6 year old. I can finally return to school, but is nursing school too difficult with a full time family and a fulltime job? I am thinking about taking a 19 month night course to become a LVN. While working days 7am-4pm.
